Forms

The Office of General Counsel has developed the following forms for customization and use by ACU faculty and staff.  It is strongly recommended that faculty and staff read the forms carefully to determine if they are appropriate for the intended use.  Please direct any questions regarding the forms to the Office of General Counsel.  To properly save the information inserted into any of these forms, do not save the form using your web browser.  Instead, after completing the form, save the document by clicking on the Save icon on the Adobe toolbar immediately above the form (it looks like a computer disk).

  • Assumption of Risk, Release and Indemnification Agreement (“Waiver”)
    This form can be used to inform individuals of the risk related to participating in certain ACU-sponsored activities and reduce ACU’s liability if the individual is injured or injures someone else during the activity.
  • Facilities Use Agreement Form
    This sample form illustrates the basic provisions and requirements of a facilities use agreement and may be used by departments with oversight of ACU facilities as a resource in negotiating or drafting such an agreement.  To submit a contract for execution by an authorized university representative, please follow the university contracting procedures.
  • Services Contract Form
    This sample contract illustrates the basic provisions and requirements of a service contract and is intended to be used by departments solely as a resource in negotiating or drafting a contract.  To submit a contract for execution by an authorized university representative, please follow the university contracting procedures.
  • Speaker Contract
    This sample contract illustrates the basic provisions and requirements of a speaker contract and is intended to be used by departments solely as a resource in negotiating or drafting a contract. To submit a contract for execution by an authorized university representative, please follow the university contracting procedures.
  • ACU Consulting Agreement
    This form can be used whenever an agreement that involves consulting services is being made between ACU and an outside party. To submit a contract for execution by an authorized university representative, please follow the university contracting procedures.
  • Assumption of Risk Release and Indemnification Agreement (ACU Sponsored Travel)
    This form can be used to inform individuals of the risk related to participating in certain ACU-sponsored travel and reduce ACU’s liability if the individual is injured or injures someone else during the activity..
  • Non-Disclosure Agreement Form
    This form may be used to enter into an agreement between ACU and a third party in which private matters or information are being shared that either party wishes to keep confidential.
