ACU Student Immunization Policy and Submission Instructions
Texas law (https://www.dshs.texas.gov/immunize/school/college-requirements.aspx) states that all students under the age of 22 enrolling for classes are required to show proof of an initial meningococcal vaccination or a booster dose during the five-year period prior to enrolling. Students must get the vaccine at least 10 days before the semester begins. There are also several vaccines highly recommended for college students. Authorization for Treatment of a Minor 
Any incoming student that is not 18 years old by the time he/she arrives on campus must have this authorization completed by a parent or guardian before the student can be seen in the clinic.

Authorization for Release of Clinical Record Information 
A release form will need to be completed by the patient/student for the clinic to release information to another physician, agency, person unless otherwise authorized by law. If you have any questions on completing the release form, please contact the clinic at 325-674-2625.

New Patient Registration 
All new patients with a myACU account may make their appointment online here (https://acu.medicatconnect.com) and will be prompted to complete new patient forms. If you are a spouse or dependent of a student or ACU employee, you may download and print these forms, complete them, and bring them with you to your appointment to expedite your visit.
