MENINGITIS B IS NOT THE STATE MANDATED VACCINE
Examples of the Texas state mandated meningococcal meningitis vaccines are MCV4 or MPSV4, sometimes known as Menactra, Menveo or Menomune.

Required Immunization: Meningococcal Meningitis
In accordance with the Texas Minimum State Vaccine Requirements for College Entry, all new, transfer, readmitted, graduate and special students and students enrolling following a break of at least one long semester are required to provide documentation of meningococcal meningitis immunization to the Admissions and Graduate Admissions Offices.

The meningococcal meningitis vaccine or booster must have been administered during the five-year period preceding and at least ten (10) days prior to the first day of the first semester in which the student initially enrolls at an institution.
The immunization date on the record must be:
between August 29, 2017 and August 19, 2022 if entering the Fall 2022 semester 

Exemption: Students who are enrolled only in online classes or are at least 22 years old are not required to receive the meningococcal meningitis vaccine.

Claim a medical exemption or exemption based on conscience

COVID-19 Vaccine (1 dose - Johnson and Johnson or 2 dose series - Moderna or Pfizer)
TDaP Vaccine within the last 10 years
MMR (2 shot series)
Annual Flu Vaccine
Hepatitis A (2 dose series)
Hepatitis B (3 dose series)
HiB (4 dose series)
HPV (3 dose series)
Meningitis B (2 shot series)
Pneumonia (4 dose series)
Polio (4 dose series)
Varicella (2 dose series or history of disease) 

Proof of Vaccination
Students may show proof of vaccination against meningococcal disease by providing their college or university with an immunization record that shows they have been vaccinated against meningococcal disease in the last five years. Acceptable proof includes one of the following:

• A form showing the signature or stamp of a physician or his/her designee, or public health personnel that shows the month, day, and year the vaccination dose or booster was administered
• An official immunization record generated from a state or local health authority that shows the month, day, and year the vaccination or booster dose was administered
• An official record received from school officials (includes records from out of state)

Students may get a copy of their immunization records from their private healthcare provider or local health department, depending on where the vaccination(s) were administered. If the student's immunization records are in ImmTrac2, the Texas Immunization Registry, he or she can request a copy by calling the Texas Immunization Information Line at 1-800-252-9152. Please note that individuals 18-26 years of age must re-consent as an adult in order to stay in the registry.

Be certain that your name, date of birth, and Banner/Student ID number appear on each sheet and that all forms are submitted together. The records must include the dates of vaccine administration – including the month, day, and year. All records must be in English. Please keep a copy for your records.

Submit your immunization forms digitally:
Go to Quicklinks on your myACU account and choose “Medical Clinic Patient Portal” and complete the following steps:

• Click on Upload tab
• Follow the onscreen instructions to upload your documentation
• Make sure you choose “Immunization Record” as the documentation type for all immunization records and/or exemption affidavits.

IMPORTANT – According to ACU policy, these requirements must be met and submitted by December 15 for students enrolling in the spring semester and August 1 for students enrolling in the fall semester.
